AHA SPRING ATHLETIC REGISTRATION IS OPEN

Physical Examination

Students registering for Activities must have a current physical examination form submitted for the Activities Trainer’s review before the student is authorized to participate in practice or tryouts. Physicals are valid for 36 months from the date of the exam. You will be prompted to upload this physical during your registration. 

Concussion testing Info: All students must have a valid baseline concussion test on file.  Tests are valid for 2 years. 
 

Testing Dates:
March 6th at 3pm
March 7th at 3pm
March 11th at 3pm

March 13th at 3pm
 

Tests take approximately 45 minutes.  Bring your chrome book to use for the test.  Contact Peter Hoffer if you are unsure if you need an updated baseline test on file.
